ADMISSION REQUIREMENTS

  • PSA copy of Birth Certificate (original and photocopy)
  • Baptismal Certificate – Roman Catholic Rites (original and photocopy)
    • For Non-Catholics – Certificate of Membership in your Religious Sect
  • 2 x 2 Colored Picture (3 copies)
  • Photocopy of latest report card with Learner Reference Number (LRN) (2 copies)
  • Certificate of Good Moral Character
  • Testing Fee – Php 500.00

ADDITIONAL REQUIREMENTS FOR NON-FILIPINO CITIZENS

1. Alien Certification of Residence (ACR) issued by the Bureau of Immigration and Deportation

2. Study permit issued by the Department of Education

APPLICATION PROCEDURE

ONLINE APPLICATION

  • Accomplish the ONLINE APPLICATION FORM found below
  • Upload the documents needed for initial evaluation of your application(PSA Birth Certificate, Latest Copy of Grades)
  • After the form and the documents have been submitted, wait for the verification email from the DBS Admissions Office for the links to the next steps in the application process.

ONSITE APPLICATION

  • Present admission requirements to the Registrar’s Office and accomplish the Application Form.
  • Pay the testing fee to the Accounting Office.
  • Present official receipt and submit accomplished application form to the Registrar’s Office.
  • Get test schedule and examination permit.
  • Take the entrance test and attend the interview as scheduled.
